Visiting Nurse Services
The CFVH Visiting Nurse Program brings a wide range of healthcare services into Sanders County homes when ordered by a physician. Most hospital patients want to go home as soon as possible. A Visiting Nurse is a sensible alternative to a long-term stay in the hospital or nursing home when the situation allows. Visiting Nurse services provided at home involve the patient and family to develop a plan of care that meets the patient's individual needs. Healthcare in the safety and comfort of the home becomes a reality with this program.
For your convenience, your insurance company will be billed directly. If there is a service that you want which is not covered by insurance, any additional cost will be discussed with you and your family in advance.
CFVH provides many visiting nurse services in the home, including:
- Skilled nursing
- Medication management
- Diabetic Management
- Disease Management
- Wound care and dressing changes
- Cardiac and respiratory assessments
- Catheter Care Management
Your Medical Provider personally supervises your care by developing a plan for your treatment. He/she will receive detailed reports on your condition from the Visiting Nurse staff.
What is Visiting Nurse?
A visiting nurse offers a complete range of skilled nursing care in your home under the direction of your medical provider. Visiting Nurse Services are available to anyone who is considered homebound. Homebound means a normal inability to leave home, requiring a considerate and taxing effort to leave.
To be eligible to receive insurance reimbursement for services, there must be a skilled medical need and it must be a taxing effort for a person to leave home. Anyone can request help - a friend, a relative, the Medical Provider or the client but a physician’s referral is needed to begin services. A nurse will make an initial visit to assess your care needs and determine whether you meet the guidelines for Medicare or insurance coverage. Before treatment is given, your medical provider is contacted. Your Provider will write the order for your care plan. Patients need to keep their medical appointments and have a provider in order to continue with the program.
