Swing Bed/Transitional Care

There may come a point during an inpatient's stay in which they no longer require acute care services. Clark Fork Valley Hospital offers swing bed care, which is a short-term transition option for patients who are not quite ready to go home but no longer require the more intense care provided in an acute care setting. This allows the patient the opportunity to continue their recovery in a safe and monitored environment.

During this transitional period, the patient will be required to:

• Dress in their own clothing from home
• Sit up in a chair for all meals
• Follow a specific treatment plan outlined by their CFVH care team
• Have reduced visits from their physician

All of these efforts will be an attempt to prepare the patient for a successful transition home.
