Social Services

Our medical social workers are trained to help patients deal with the social and emotional challenges of a hospital admission and discharge. They offer a wide range of services when a patient is in the hospital, for both them and their family members, including counseling, information, advance medical directives forms and even suggestions for overnight accommodations for out-of-town visitors.

Social Services, along with our Nursing staff, will assist you in making the arrangements for your discharge home or your transfer to another facility. This may include home health or other supportive care that is part of your doctor’s follow-up plan, as well as home medical equipment (such as walkers or oxygen) that you may need.
