Advance Healthcare Directive Event - Plains
Advance directives are legal documents that allow you to spell out your decisions about end-of-life care ahead of time. They give you a way to tell your wishes to family, friends, and health care professionals and to avoid confusion later on.
The purpose of this event is to eliminate the intimidation of creating an Advance Directive by working through it, and explaining each step in a group process. Attendees need to bring identification and the names and contact information of their Medical Power of Attorney(s). The Medical Power of Attorney is the person you choose to advocate for your health decisions in the event you are unable to speak for yourself.
During the event, we will complete the Advance Directive, notarize it making it a legal document, and make copies (for the medical record and for distribution to appropriate people/places).
