Charge Nurse RN/LPN-Long Term Care

Job Description

SUMMARY Provides general nursing care to patients in nursing home by performing the following duties:

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Resident Responsibilities: Conducts assigned assessments, including assigned parts of MDS and completes new admissions assessments as assigned (RN only). Monitors assessments assigned to CNA's. Assess resident response to interventions, treatments and medications, modifies plan of care (RN) based upon assessments, and develops new plans of care as indicated (RN). Notified medical provider of resident status and changes promptly. Interacts with residents, families and other staff and departments in professional manner. Maintains nursing skills at high level, including infection control measures. Participates in QA/PI activities as assigned. Use software system appropriately. Interacts with resident individually in most professional and compassionate manner. Communicates with medical providers regarding changing resident status or concern and notifies medical provider of resident status and changes promptly.

Medication Responsibilities: Accurately administers medications, re-orders, completes documentation, and checks in delivered medications. Carries out ordered treatments, and assures ordered diagnostic tests are ordered and completed. Maintains resident medical records in complete and accurate manner. Completes job duties of assigned shift and communicates resident information to on coming shift in a  thorough manner. Completes accurately all medical provider orders, entering information in the computer system, communicating the order appropriately to other staff.

Charge Functions: Makes CNA's Assignments. Communicates with CNA regarding resident daily needs and receives report from CNA's. Makes rounds with medical provider. Supervises CNA's daily performance and contributes to CNA evaluation as required. Participates in QA/PI activities and participates in revisement of Policies/Procedures. Receive, record and replace staff call off/sick calls. Notify Manager, Resident Care Coordinator or Administrator of unusual occurrences/incidents as appropriate.  Complete appropriate documentation for occurrence report and workers compensation reports/resident reports.

Requirements

CERTIFICATES, LICENSES, REGISTRATIONS Graduate of an accredited school of nursing; currently licensed in the state of Montana as a Registered Nurse or Licensed Practical Nurse; maintain CPR certification.

EDUCATION and/or EXPERIENCE Minimum of Diploma or Associate's degree (A.A.) as an LPN or RN; prefer one to two years related experience and/or training.
