Job Description

SUMMARY Provides skilled nursing visits to Home Health and Hospice patients in the home setting under the direction of the nurse case manager assigned to the patient.  This is accomplished by the utilization the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation.  

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.   Provides skilled nursing care to patients and families based upon a systematic evaluation, directed by nurse case manager and as authorized by a physician.   Responsible for pertinent observation and evaluation of physical and psychological status including changes in condition and or reactions to treatments and medications.   Assures continuity of care through effective communication and information exchange and or case conferences with other disciplines, community health, support and social agencies.   Promotes optimal health care and quality of life through teaching, counseling appropriate interventions and referrals.   Responsible for assigned skilled nursing Home Health and Hospice care visits, assigned continuous care hours, limited on-call coverage and crisis management.

CERTIFICATES, LICENSES, REGISTRATIONS Current Montana License- Registered Nurse Current CPR

EDUCATION and/or EXPERIENCE   Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
